COVID-19 Screening
If your child is arriving unaccompanied to the program site, the below screening questionnaire must be completed before the child is admitted into the program. A participant with one or more symptoms of COVID-19 should not report to the program. In the event a child arrives to a program site and the screening has not been completed, the parent will be contacted and asked to submit, before the child is allowed to participate. If a parent or participant needs more information about COVID-19, finding a medical provider, or accessing COVID-19 testing, please call 311. If a participant is experiencing a medical emergency, they should call 911.
